DSM-V Diagnostic Criteria for Insomnia
Insomnia is listed as a sleep-wake disorder in the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-V). The latest edition of DSM-5 defines insomnia as “dissatisfaction with sleep quantity or quality, associated with one (or more) of the following symptoms:
- Difficulty initiating sleep
- Difficulty maintaining sleep, characterized by frequent awakenings or problems returning to sleep after awakenings
- Early morning awakening with the inability to return to sleep
As per the American Sleep Association (ASA), almost 70 million Americans suffer from one or another type of sleep disorder. Nearly 40% of individuals fall asleep during the day and 5% of people fall asleep while driving.
DSM-V mentions the following criteria for the diagnosis of insomnia disorder:
- Dissatisfaction with the quality or quantity of sleep includes difficulty falling asleep, remaining asleep, or waking up early, and then the inability to fall asleep again.
- This sleep deprivation causes a great amount of stress and impairs social, occupational, educational, academic, behavioral, or other important areas of functioning.
- Difficulty in sleeping occurs at least three nights per week and is present for at least 3 months.
- The sleep difficulty occurs despite several opportunities to fall asleep.
Insomnia cannot be better explained by any other physical, mental or sleep-wake disorder such as narcolepsy, a breathing-related sleep disorder, or a sleep-wake disorder. - Mental disorders and medical conditions cannot adequately explain the insomnia experience.
- The sleep disability does not occur due to any substance use or medication.
Healthcare providers might recommend a physical examination and blood tests to check for thyroid function and any other disorders. They may also use Sleep diaries and the Epworth Sleepiness Scale to better diagnose insomnia.
A sleep diary helps doctors detect changes in sleep patterns, and the Epworth Sleepiness Scale identifies how tired or drowsy a person feels when confronted with specific situations, such as waiting in a long queue, reading or watching television.

FAQ
How Often does Insomnia Occur

It affects a significant portion of the population worldwide. Various studies worldwide have shown the prevalence of insomnia in 10%–30% of the population, some even as high as 50%–60%.
It is commonly observed in older adults because the prevalence of insomnia increases with age. Women are more likely to experience insomnia than men due to hormonal fluctuations and stress factors. It is important to consult a healthcare provider to get a personalized insomnia treatment tailored to individual needs.

Can Antibiotics cause Insomnia?
Yes, antibiotics can cause insomnia, but the connection isn’t so simple. Antibiotics are prescribed to fight bacterial infections, yet some people notice trouble falling or staying asleep while taking them. This doesn’t happen to everyone, but it’s real for many and worth understanding.
Some antibiotics, especially certain fluoroquinolones and tetracyclines, have been associated with nervous system stimulation or mild side effects that can interfere with restful sleep. Others may affect the gut microbiome, the community of bacteria that influences not only digestion but also mood and relaxation pathways. When that balance shifts suddenly, it can reduce deep sleep.
It’s also true that the fever, pain, inflammation, or discomfort often results in poor sleep. When your body is fighting infection, your nervous system stays alert, making deep sleep harder to achieve. In some cases, antibiotics may interact with other medications you’re taking, creating a stimulant effect.
Antibiotic-related insomnia is usually temporary, and sleep often improves once the medication course ends and the infection clears. If sleep problems persist, checking in with a psychiatrist can help ensure that nothing deeper is contributing. Early support can make the difference between restless sleep and real rest.
